To create an account for the Northwell Health patient portal, follow these steps:
Go to the Northwell Health patient portal login page at https://mychart.northwell.edu/MyChart/. Click on the “Sign Up Now” button located below the login fields.
- On the next page, you will be asked to verify your identity. Choose one of the options available: “MyChart Activation Code”, “Social Security Number”, or “Medical Record Number”. Enter the requested information and click “Next”.
- If you choose “MyChart Activation Code”, you will need to enter the code provided to you by your Northwell Health provider. If you choose “Social Security Number” or “Medical Record Number“, you will need to enter your personal information as requested.
- After verifying your identity, you will need to create a MyChart username and password. Your username must be unique and at least six characters long. Your password must be at least eight characters long and contain at least one uppercase letter, one lowercase letter, and one number.
- After you set up your username and password, you’ll be asked to choose and answer three security questions.If you ever forget your login information, these questions will be used to help you get back into your account.
- Review the terms and conditions of use and the privacy policy, and then click the “Accept” button to complete the registration process.
Congratulations! You have successfully created your Northwell Health patient portal account. You can now log in to your account and access your health information, make appointments, and communicate with your healthcare providers.
